Keratoconus is a deformation of the cornea that usually occurs between the ages of 20 and 30 and compromises the possibility of leading a normal life. It is a degenerative and debilitating corneal disease that affects 1 in 500 people. In 96% of cases, it is bilateral and occurs at a young age, generally between 20 to 30 years old. The causes are still unknown. In some cases, there is a hereditary component, while in predisposed people, it is thought that the use of contact lenses or continuous rubbing of the eyes may cause it. If the keratoconus is not stopped with corneoplasty, corneal transplantation occurs in 22% of the patients within 7-8 years of the first diagnosis.

“Early diagnosis and targeted treatments are the keys needed for good therapeutic success and to maintain the best quality of life,” said Dr. Alberto Bellone. Keratoconus is a disease characterized by a progressive thinning and deformation of the central area of ​​the cornea which presents an irregular cone-shaped bulge. It is a subtle disease. Initially, it is often asymptomatic and confused with other visual defects such as myopia and astigmatism. “The need to increase the gradation of the lenses frequently is an alarm bell. The discovery of suffering from keratoconus, then, can have a very strong psychological impact, because it manifests itself at a young age when one has a long life expectancy. Keratoconus, while not leading to blindness, does not allow one to lead a normal life,” warns Bellone.

For this reason, we should regularly undergo the appropriate diagnostic tests such as corneal altitudinal topography, pachymetry and corneal tomography, which are simple, fast and painless from 5-6 years of age for children who have a family history of keratoconus as explained by Bellone.

As for the treatment of the disease, there are currently no pharmacological therapies that can restore the infected eye to a condition with the absence of keratoconus. “However there are corneoplasty treatments able to stop the progression of the disease and allow the patient to live a normal life”, explains Bellone. We are talking about corneal cross-linking and implant of intrastromal rings (Kerarings) to which a subsequent implant of phakic intraocular lenses will be added. “Every treatment strategy must be highly personalized. The important thing is to not have to reach corneal transplant which is an extreme solution that can be avoided with early diagnosis and valid and tested treatments. Let’s not allow an initially small problem, if underestimated, become a serious problem,” concludes Bellone.
